Tubule expansion compresses kidney vessels

A kidney hypothesis asks if blocking a sodium-glucose transporter could lower oxygen in the inner kidney.
It reabsorbs sodium and glucose from filtrate. Blockade may send more fluid downstream. In the crowded renal medulla, the inner kidney, expanded tubules could squeeze low-pressure vessels, reducing perfused capillaries and causing distal hypoxia, an oxygen shortage downstream.
The test would relieve pressure with filtration, reabsorption, temperature and arterial pressure matched. Flow, oxygenation and reserve during repeated challenges would support compression. Persistent hypoxia after confirmed decompression would reject the hypothesis. Pressure, imaging, perfusion and oxygenation could distinguish it from thermal, energetic and mitochondrial explanations.
