Chronic kidney disease can damage kidney reserve, and this proposed mechanism asks whether a treatment that protects the kidney could shift oxygen stress into downstream tubule cells during recovery. The hypothesis centres on mitochondrial coupling: mitochondria normally turn oxygen use into ATP, the cell's transport fuel. If blocking sodium-glucose cotransporter 2, a kidney transport protein, changes local substrate supply and osmotic load, protons may leak across the inner mitochondrial membrane. The cell could then consume oxygen less efficiently, producing less ATP for each unit of oxygen used. To distinguish this from local heating, oxygen competition or tissue compression, researchers would isolate downstream nephron segments and match temperature, oxygen and substrates.
