First human trial of cellular "rejuvenation" gets FDA green light

Life Biosciences, a startup co-founded by Harvard's David Sinclair, received FDA clearance on January 28 for a Phase 1 clinical trial (NCT07290244) of ER-100, a gene therapy based on partial epigenetic reprogramming. Patient recruitment is now underway, with results expected by late 2026 or early 2027.

The idea is this: three of the four Yamanaka factors, Oct4, Sox2, and Klf4, can "roll back" a cell's age without erasing its identity. The full set, with c-Myc added, turns a cell into a stem cell: it forgets what it was and can become a tumor. The trio without c-Myc reverses epigenetic marks, but the cell remains itself. ER-100 delivers those three factors via an AAV2 vector, and the therapy is activated with a standard course of doxycycline: 8 weeks of pills. In primate experiments, Life Biosciences induced optic nerve damage and then restored vision to the level of a healthy animal.

They are starting with the eye: glaucoma and ischemic optic neuropathy (NAION). Glaucoma is the second leading cause of blindness worldwide. NAION is entirely untreatable: there are no approved therapies, and the odds of losing the second eye within three years are 20-30%. The eye is a practical first testing ground: an injection delivers the therapy directly to damaged retinal cells, without the need for systemic delivery.

Context. Altos Labs (Bezos) raised $3 billion back in 2022 and is still not in the clinic. NewLimit (Armstrong, Coinbase) raised $130 million and is also still preclinical. Life Biosciences, a team of fewer than 20 people, got to humans first. That does not mean they will be the first to succeed, but they are the first to put the technology up against reality.

Patient recruitment will take place over the coming months. If the therapy shows even partial vision restoration, it will be the first proof that epigenetic reprogramming works in humans, not just in mice and monkeys. That would change the conversation, both for glaucoma and for aging more broadly.
