Noninvasive intermediate-frequency alternating electric fields (Tumor Treating Fields, TTFields, 150–200 кГц) cause focal nuclear-envelope disruption and cytosolic micronuclear clusters in dividing glioblastoma cells, converting the tumor microenvironment from immunosuppressive to immunopermissive and increasing survival in patients with glioblastoma receiving maintenance temozolomide.

Literature

Partial results published; complete hypothesis not found

Chen and colleagues directly report the central nuclear-envelope, DNA-sensing, and antitumor-immunity mechanism, including experiments in KR158-luc. However, their implantation study uses in vitro TTFields pretreatment and dual knockdown with scrambled controls; the specified double-knockout and sham-heat comparison is not reproduced. Their separate temozolomide experiments do not supply that missing design. Human immune profiling also does not establish that this mechanism causes the survival benefit during maintenance temozolomide. The supplied literature therefore supports substantial components, but neither confirms nor contradicts the complete proposed test.

What is new. The unmatched extension is the specified KR158-luc experiment combining wild type versus STING/AIM2 double knockout with TTFields versus sham-heat control and an explicitly positioned temozolomide-monotherapy comparison, together with demonstration that the proposed immune mechanism mediates patient survival benefit. The central mechanism and KR158-luc immune effects are already reported. These gaps concern the supplied literature and do not establish universal novelty.

Comparison of the complete hypothesis
  1. The intervention uses noninvasive intermediate-frequency alternating electric fields, termed Tumor Treating Fields (TTFields), at 150-200 kHz.

Matches this study. The study uses noninvasive alternating electric fields and applies 200 kHz to glioblastoma cells, within the specified range. Its 150 kHz experiments concern lung and pancreatic cancer cells.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Abstract; Results, Figure 1A and accompanying text; Supplemental Figure 4 description.)

  2. TTFields cause focal nuclear-envelope disruption and the formation of cytosolic micronuclear clusters in dividing glioblastoma cells.

Matches this study. Microscopy demonstrates focal nuclear-envelope rupture and cytosolic micronuclear clusters in glioblastoma cells. Ribociclib experiments establish dependence on cell-cycle entry; affected cells need not be in mitosis.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figure 1A-D; Supplemental Figures 1-7 descriptions.)

  3. Nuclear-envelope disruption and cytosolic micronuclear clusters recruit and activate the cytosolic DNA sensors cGAS/STING and AIM2/caspase-1 inflammasomes.

Matches this study. The study demonstrates sensor recruitment to micronuclear clusters, STING-dependent inflammatory signaling, and AIM2-dependent caspase-1 activation, supported by depletion and rescue experiments.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figures 1-5; STING and AIM2 depletion and rescue experiments.)

  4. This signaling converts the immunosuppressive tumor microenvironment into an immunopermissive state, with activation of dendritic cells and adaptive CD8+ T-cell immunity.

Matches this study. Conditioned-medium and syngeneic implantation experiments demonstrate dendritic-cell activation, activated CD8+ T cells, tumor immune infiltration, and memory responses dependent on tumor-cell STING/AIM2 expression. These animal results follow implantation of cells pretreated with TTFields in vitro.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figures 6-10; Figures 7E-F and 8D-E.)

  5. The proposed TTFields-induced mechanism increases survival in patients with glioblastoma receiving maintenance temozolomide.

Not verified in the article. The 12-patient cohort measures immune changes after initiation of TTFields and maintenance temozolomide, rather than testing whether this mechanism mediates improved survival. The survival trial is cited, but its full article is absent from the supplied sources, and the causal mechanism-to-survival link remains unverified.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figures 11-15; Discussion; reference 81.)

  6. The decisive experiment uses the syngeneic KR158-luc glioblastoma model.

Matches this study. Luciferase-tagged KR158 cells are used in an orthotopic glioblastoma model syngeneic to C57BL/6J mice, matching the specified model.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figures 6, 7 and 9; Table 1.)

  7. The decisive experiment is specified as a 2×2 design comparing wild type with STING/AIM2 double knockout (DKO), crossed with TTFields versus a sham-heat thermal control and temozolomide monotherapy; the placement of temozolomide monotherapy within the stated 2×2 design remains unspecified.

Different experimental conditions. The four groups cross scrambled-control versus dual STING/AIM2 knockdown with untreated versus TTFields-pretreated cells. Knockdown substitutes for the specified knockout, and a sham-heat control is not verified. Temozolomide alone and with TTFields is tested separately using 300 μM for 72 hours before implantation, rather than within the specified knockout factorial experiment. The proposed placement of temozolomide within 2×2 remains ambiguous.

Tumor Treating Fields dually activate STING and AIM2 inflammasomes to induce adjuvant immunity in glioblastoma (Results, Figure 7A-D; Supplemental Figure 15 description; Methods.)

Selective inhibition of the persistent sodium current of the voltage-gated channel NaV1.5 (canonical target SCN5A), aberrantly expressed in human breast cancer cells, using the approved phase 4 drug ranolazine (50 мкМ in vitro; 50 мг/кг/сут in vivo) eliminates pathological membrane depolarization, suppresses proteolytic matrix degradation, and reduces metastatic lung colonization without cardiotoxicity.

Mechanism

Ranolazine selectively inhibits the persistent sodium current of NaV1.5, eliminating pathological membrane depolarization and suppressing proteolytic matrix degradation.

Prediction

Literature

Partial results published; complete hypothesis not found

The central anti-invasive and lung-colonization effects of ranolazine were reported by Driffort and colleagues in 2014. Their study also linked these effects to NaV1.5 through genetic suppression and in vitro combination testing. However, it used a three-group tail-vein model, not the proposed four-group orthotopic experiment in NMRI nude mice, and did not document mitochondrial β-oxidation control, QT monitoring, or cardiac-specific safety outcomes. It also did not establish the complete persistent-current-to-depolarization-to-proteolysis causal chain. Nelson and colleagues provide separate orthotopic evidence for NaV1.5 suppression, while Yang and colleagues connect persistent sodium current to depolarization and migration using other interventions. The latter study distinguishes voltage-dependent migration from voltage-independent invasion, qualifying the proposed mechanistic link without refuting the complete experiment. No single supplied primary study performed the whole specified test.

What is new. The unmatched extension is the integrated four-group test in NMRI nude mice with orthotopic MDA-MB-231 xenografts, including SCN5A suppression plus ranolazine, the stated dosing regimen, mitochondrial β-oxidation control, and QT monitoring, together with direct verification of the complete causal mechanism and cardiac safety. Ranolazine-mediated inhibition of invasion and reduction of lung colonization are already reported components. The supplied literature does not establish whether this complete extension has been tested elsewhere.

Comparison of the complete hypothesis
  1. The biological target is the voltage-gated sodium channel NaV1.5, with SCN5A as its canonical target identifier, aberrantly expressed in human breast cancer cells.

Matches this study. Driffort and colleagues studied functional NaV1.5 in human MDA-MB-231 breast cancer cells, measured SCN5A expression, and suppressed its expression with targeted shRNA.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Findings; Figures 1 and 2A-B.)

  2. The intervention uses ranolazine, described as an approved phase 4 drug, at 50 µM in vitro and 50 mg/kg/day in vivo.

Different experimental conditions. The study used 50 µM ranolazine in vitro and 50 mg/kg intraperitoneally in mice, but administered the animal treatment five days per week. It identifies ranolazine as approved for chronic angina; the phase 4 designation is not documented.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Findings, ranolazine pharmacology and animal experiment; Figures 1, 2E-G and 3A.)

  3. Ranolazine selectively inhibits the persistent sodium current of NaV1.5, thereby eliminating pathological membrane depolarization and suppressing proteolytic matrix degradation.

Not verified in the article. Ranolazine inhibited sodium currents and reduced focal matrix proteolysis, with an invasion assay supporting NaV1.5 dependence. However, the reported electrophysiology measured peak currents and gating; it did not establish selective persistent-current inhibition followed by elimination of pathological membrane depolarization as the cause of reduced proteolysis.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Findings; Figure 1A-C and Figure 2E-G.)

  4. Ranolazine treatment is predicted to reduce metastatic lung colonization without cardiotoxicity.

Not verified in the article. Ranolazine reduced the lung bioluminescence signal by 77 ± 8% relative to control, supporting reduced colonization burden. Similar body-weight trajectories and the authors' description of no apparent toxicity do not establish absence of cardiotoxicity; cardiac-specific measurements are not reported.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Findings, lung colonization results; Figure 3B-E; concluding discussion.)

  5. The decisive experiment uses NMRI nude mice bearing orthotopic MDA-MB-231 xenografts.

Different experimental conditions. The ranolazine study used NMRI nude mice and MDA-MB-231-Luc cells, but injected cells into the tail vein rather than establishing orthotopic xenografts. Its three groups contained 18, 12 and 8 mice, totaling 38. Nelson and colleagues separately studied orthotopic MDA-MB-231 tumors in Rag2−/− Il2rg−/− mice.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Findings, animal experiment; Figure 3A-E.)

  6. The decisive comparison includes four groups: control, ranolazine, SCN5A-targeting short hairpin RNA (shRNA), and SCN5A-targeting shRNA combined with ranolazine.

Different experimental conditions. The animal experiment compared control cells, SCN5A-suppressed cells, and control cells with ranolazine; it omitted the combined SCN5A-suppression plus ranolazine group. That combination was examined in vitro, where ranolazine produced no further reduction of invasion after NaV1.5 suppression.

Ranolazine inhibits NaV1.5-mediated breast cancer cell invasiveness and lung colonization (Figure 2E; Findings, animal experiment; Figure 3A-C.)

  7. The experiment includes control of mitochondrial β-oxidation, with the form of that control left unspecified.

Not verified in the article. No supplied study documents control of mitochondrial β-oxidation within the proposed ranolazine experiment. The separate metabolic study measured glycolysis and mitochondrial respiration under other interventions, which does not verify this β-oxidation control.

  8. The experiment includes electrocardiographic monitoring of the QT interval.

Not verified in the article. No supplied study reports electrocardiographic QT monitoring in the proposed breast cancer experiment.

H3Combined suppression of the sympathetic neuro-tumor circuit through optogenetic inhibition of sympathetic fibers using Jaws / HcKCR1 rhodopsins and pharmacological blockade of ADRB2 with the approved phase 4 drug propranolol blocks neoangiogenesis and suppresses tumor metastatic potential in animals and patients.
Mechanism and prediction

Mechanism. Sympathetic adrenergic nerve fibers growing into the microenvironment of solid tumors release noradrenaline and activate ADRB2 (β2-adrenoceptors) on endothelial and tumor cells, triggering an angio-metabolic switch and epithelial–mesenchymal transition (EMT). Optogenetic inhibition of these fibers combined with ADRB2 blockade suppresses this circuit.

Prediction. A 2×2 experiment in an orthotopic breast/prostate cancer model: HcKCR1 optogenetic nerve inhibition × systemic propranolol versus selective Adrb2 knockout in the endothelium and tumor, separating the local neuronal contribution from the systemic action of the β-blocker.

H4Targeted inhibition of ATP1A1 (the α1-subunit of Na+/K+-ATPase) with the approved phase 4 drugs digoxin or ouabain, combined with optogenetic modulation of membrane polarization, induces apoptosis exclusively in senescent human cells and suppresses the inflammatory SASP secretome in vivo without a risk of tumorigenesis.
Mechanism and prediction

Mechanism. Senescent human cells have a chronically depolarized membrane potential, mediated by SCN9A and the Ca2+/Rb pathway that keeps them from oncogenic division, and increased sensitivity to disruption of transmembrane Na+/K+ and H+ gradients. ATP1A1 inhibition combined with optogenetic modulation causes selective depolarization and osmotic overload, inducing the proapoptotic protein NOXA and senolysis.

Prediction. A 4-group comparison in aging mice and an induced-senescence model: control / digoxin / optogenetic Vm shift / combination with forced expression of the K+ channel HcKCR1, maintaining hyperpolarization. The experiment tests whether maintaining K+ hyperpolarization prevents digoxin's senolytic effect, excluding a Vm-independent effect of cardiac glycosides, while monitoring SASP and the absence of clonal proliferation of damaged cells.

H5A two-component noninvasive optogenetic system (an “optical resting-potential clamp”) combining systemically delivered (AAV-PHP.eB/AAV9) red-shifted cation channel ChRmine and highly sensitive K+-selective channelrhodopsin HcKCR1-hs allows noninvasive switching of Vmem in deep tissues between depolarized and hyperpolarized states without shifting intracellular pH, suppressing pathological seizure activity and causing regression of oncogene-induced tumors.
Mechanism and prediction

Mechanism. ChRmine (585 нм, depth up to 7 мм) and HcKCR1-hs (PK/PNa ≈ 23, spectrum up to 630 нм) are optimized by machine learning algorithms for photocurrent magnitude and inactivation rate. Transcutaneous illumination in the orange-red range (≥0.1–0.8 мВт/мм2) switches Vmem between depolarized and hyperpolarized states without shifting intracellular pH.

Prediction. Pathological seizure activity is suppressed by a factor of 3.8 (from 3.29 to 0.86 generalized seizures), and oncogene-induced tumors regress. The experiment includes an mCherry thermal control and comparison with NHE3/Arch, with simultaneous recording of Vmem and pHi.

H6Controlled hyperpolarization/modulation of membrane potential Vm in passaged human articular chondrocytes on the PixCells platform stabilizes SOX9 and enhances the nuclear response of TGF-β/SMAD2/3, restoring collagen II and aggrecan synthesis. Subsequent delayed dynamic compression of the 3D construct maintains an increased Young's modulus after optical stimulation stops.
Mechanism and prediction

Mechanism. Vm modulation through K+ and Cl− channels regulating cell volume and intracellular Ca2+ oscillations stabilizes the master regulator of chondrogenesis SOX9 and enhances the nuclear response of the TGF-β/SMAD2/3 cascade. Delayed dynamic compression consolidates the increased Young's modulus.

Prediction. A paired 2×2×2 factorial experiment (Vm × TGF-β1 × dynamic compression), excluding four alternatives: an effect of TGF-β1 alone; a thermal/ionic artifact of a single tool; increased markers without mechanical improvement; and irreversible replicative senescence of passaged chondrocytes assessed by p16/SA-β-gal.
