Japan Times: Japan responds to its caregiver shortage with AI companions for older adults

In Japanese nursing homes, older adults are being given plush robots and conversational AI dolls are being tested as the country faces a potential shortfall of 570 thousand caregivers by 2040
On April 30, Japan Times republished a Bloomberg column on Japan’s care institutions. The author saw baby-sized robots that respond to touch, along with prototype conversational dolls for lonely older adults.

After his wife died, her 97-year-old grandfather rejected ChatGPT’s advanced voice mode: he heard the upbeat synthetic voice and said he had no desire to talk to it. For someone who has lost a spouse, artificial friendliness can sound like a poor substitute for a real conversation.

Then the math takes over. By 2040, Japan’s care sector could be short 570 thousand workers, the people who lift residents out of bed, help them wash, remind them to take medication, notice a fall in the night, and simply talk to them. Families are smaller, the elderly population is larger, and caregivers do not get extra hours added to the workday.

Japan is showing its own version: filling the labor gap with robots and conversational AI devices right in the care room.

Japan has already spent money on care robots. At one facility, Thorbecke saw SoftBank Pepper: once marketed as a symbol of the robotized future, the now-discontinued robot was standing in a corner leading exercises next to a staff member repeating the same movements. In the book Robots Won’t Save Japan, ethnographer James Wright described the same side effect: staff had to switch the robots on, move them around, explain them to residents, and fix malfunctions, so the machine sometimes created more work.

The current wave adds conversational AI. South Korea has distributed about 14 thousand Hyodol devices: soft dolls for lonely older adults that talk, give reminders, and send an alert if a person has not moved for a long time. In China, retirees ask ByteDance chatbot Doubao how to make sense of small print in an instruction manual, while Tencent is teaching older adults to use Yuanbao for prescriptions and everyday questions.

The evidence on robots in care is stronger where the machine takes on a narrow physical or monitoring task. Yong Suk Lee of the University of Notre Dame describes the mechanism this way:

“Robots can raise productivity by freeing staff for tasks that require human touch, empathy, and dexterity.”

In these data, robotization was associated with lower turnover, fewer bedsores, and less use of physical restraints. Robots look strongest when they take on a clear, specific task: lifting a person, detecting a lack of movement, or quickly summoning a caregiver. That leaves workers with more energy and more minutes for human care.

There is a limit to this approach. Robots for independent mobility were associated with more falls: a vulnerable person gets more freedom and a new risk at the same time. A conversational doll becomes dangerous when an institution uses it as a substitute for hiring, pay, and support for home care.

Thorbecke cites an estimate that almost every OECD country will have more than 20% of its population over 65 by 2050. Japan shows what that demographic reality looks like inside a nursing-home room: a soft robot, an exhausted caregiver, and a human shortage that can already be counted.
